JOB PURPOSE:

A Household

The successful candidate shall be responsible for providing compassionate and skilled nursing care to patients in their homes. The role reports to the Supervisor.

ROLE SPECIFIC REQUIREMENT:

• Ensure that patients receive high - quality care in the comfort of their homes.


• Conduct initial assessments of patients physical, emotional and social needs in their homes and develop and implement individualized care plans.


• Administer medications and provide other skilled nursing interventions based on patients' needs and care plans.


• Coordinate with other healthcare professionals, such as pharmacists to ensure continuity of care and propoer communication.


• Monitor patients' progress, as well as document and maintain accurate and complete patient records.


• Perform other duties and responsibilities that may be assigned by Superior.

• Must be naturally caring and industrious.


• Possess excellent communication and interpersonal skills.


• Must be trustworthy and patient.


• Must have good organizational and multitasking abilities.


• Ability to lift and transfer patients as needed.
